What tripped up Dick Van Dyke in the opening credits of his classic sitcom?

In order to keep the joke fresh, producers of the "Dick Van Dyke Show" rotated between a few different versions of the opening credits.

Instead of tripping, sometimes Dick would sidestep the ottoman at the last second.  And in yet another version, he would avoid the ottoman only to stumble over the carpet.
